The registration forms and agreements listed below are in PDF format. ALL of these forms need to be printed, read, and signed new patient registration. Please download the forms to complete at your convenience and bring them with you for your first appointment. This will save time on your appointment day and will be greatly appreciated.

Financial Options

It is important to all team members of this office that each patient has a clear picture of his or her dental treatment needs and financial responsibilities associated with them. We welcome the opportunity to discuss treatment goals with you, answer questions, and encourage open communication regarding financial arrangements. We have a genuine appreciation and fondness for our patients built on honesty and integrity and will make every effort to work out a timetable for your budget.

Financial arrangements will be willingly determined ahead of time, leaving no uncertainty once treatment begins. We anticipate a mutually beneficial relationship with all of our clients both young and old.

We have several payment options available to you for the portion of costs not covered by insurance and to help make dentistry affordable. These options are:

Insurance

Dental insurance is intended to cover some, but not all of the cost of your dental care. Since we provide dental services to you; you are ultimately responsible for all fees regardless of third party involvement. We will gladly file your insurance claims along with the proper documentation to help you maximize your dental benefits. Because we deal with numerous dental carriers, it is impossible for us to know all of the details associated with each of the particular insurance plans. It is your responsibility to know the details of your individual plan.
